今回は個人経歴開示フォームの最終回です。
複数管轄権個人経歴開示フォーム(MJPHDF)の記載内容（No75～No77）
７５では、申請者の身内以外で申請者のことを証言できる者を３名記載することが求められています。
７６は、これまでの設問の回答欄に描き切れなかったことを書くページです。
最後の７７は、これまでの設問に対する回答が真実であることの宣誓を求めるところで、申請者本人と公証人等の署名が求められています。
今後、引き続き事業者の経歴開示フォーム（MULTI JURISDICTIONAL BUSINESS FORM）を紹介していこうと思っていますが、次回からは当分の間、実際の免許の申請に対する調査が、どのような体制でどのように行われているのかについて、マサチューセッツ州で公開されている調査事績報告書に基づいてみていこうと思います。
